Meningococcal Conjugate Vaccine (MCV4)
PLEASE READ CAREFULLY
The following lists information that is required of all students in regard to the MCV4.
For Freshman, Sophomores, and Juniors: One dose is required on or after the 11th birthday.
For Seniors: One dose is required on or after the 11th birthday and a second dose after the 16th birthday.If the first dose is given after the 16th birthday, only one dose is required.No proof of immunity is allowed.
Reminder: Please only submit medical paperwork to a school nurse.

NEW! State of IL Proof of School Dental Exam
By May 15th of their freshman year, students in grade 9 are required to have a dental exam on file. School dental examinations must have been completed within 18 months prior to the May 15th deadline. Requirement begins with the freshman class of 2023.

Vaccination Information

Required health documents include a 9th grade physical must be completed within one year of enrollment on the State of Illinois Certificate of Health Examination Form, a dental examination form or waiver, and a complete immunization record including the following:
Day, month, & year of doses are required, as well as a signature and stamp by a health care provider. Doses must include:
- (3 doses) Polio with the last dose after age 4
- (3 doses) DTP/DTaP/Td with the last dose after age 4, along with a Tdap
- (3 doses) Hepatitis B
- (2 doses) Measles, (2 doses) Mumps, (2 doses) Rubella, (2 doses) Varicella {1st given on or after the 1st birthday; 2nd given no less than 4 weeks (28 days) later}
- (1) dose of the Meningococcal Conjugate Vaccine for 9th, 10th, & 11th
- (2) doses of the Meningococcal Conjugate Vaccine for 12th Only 1 dose is required if 12th graders received the 1st dose on or after their 16th birthday
