Thornton Township High Schools District 205 has partnered with Universal Dental for each schools' Oral Health Program. Services provided include an exams, cleanings, fluoride treatments, and sealants. Parents need not be present if a consent form is filled out and submitted in advance to your respective school's nurses' office. Please print and complete the consent form below and turn it in to your respective school nurses' office.
